On behalf of the residents and staff of Shawnee Rose Care Center, I would like to extend a very warm welcome to you and your loved one. The staff of Shawnee Rose Care Center strives for excellence in obtaining each resident’s individual needs and goals to attain their highest level of independence.
Our main objective is to provide excellent nursing care. We have a very dedicated and caring nursing staff to meet your needs. We also offer an exciting activity and social service department to provide for your social needs. We strongly encourage family involvement with our residents at any time.

Specialized Nursing Programs
Our rehabilitation team is made up of highly skilled and experienced physical, occupational and speech therapists who strive to improve each patient’s ability to function and care for themselves again. All of our staff members are graduates of accredited programs with national credentials and state licenses, so you can rest assured that your patient or family member is in the hands of highly qualified rehabilitation professionals. We take a progressive team approach with the therapy program specifically designed for each patient following the evaluation of his or her functional abilities. Specialized Clinical Programs
Wound Care: Offers wound evaluation, measurement, treatment and prevention programs designed to improve or preserve skin integrity.
Restraint Reduction: Decreases resident’s needs for restraints through the use of appropriate wheelchair seating, positioning and staff education.
Rehab Dining: Increases safety during dining by determining a feeding approach and swallowing precautions appropriate to individual abilities.
Dysphagia Therapy: Evaluates and treats swallowing disorders to reduce the risk for aspiration and improve nutritional intake.
Aural Rehabilitation: Identifies and refers patients in need of audiological evaluation, improves speech reading of hearing impaired and trains caregivers on hearing aid maintenance.
Older Adult Cognitive Rehabilitation: Increases staff understanding of cognitive etiology and patient learning potential by establishing three levels of functional care: restoration, compensation and adaptation.
Restorative Nursing: Designs individualized rehabilitation programs for all residents to restore and maintain physical abilities.
Functional Maintenance: Assesses and establishes a plan to ensure patient’s highest level of function by training caregivers on appropriate treatment.

Nursing
Our Nursing Team consists of compassionate, well-educated professionals whose skills and experience are focused on meeting the total care needs of our residents! Our nursing staff is continually updated and trained on the most progressive treatment options available anywhere. They are on duty 24 hours a day, 7 days a week to serve our residents and answer your questions!
Dietary
Our dietary specialists know the value of a well-prepared, appetizing meal. They are uniquely equipped to meet the specialized dietary needs of our clientele as well as accommodating a wide variety of personal preferences shaped by a lifetime of experience. Our modern and sanitary kitchen is a model for our industry. Friends and family members are encouraged to join us for delicious meals on a regular basis. There is a small charge for those wishing to join us.
Social
Our social service team in conjunction with our interdisciplinary team generates quality programs whose purpose is to fulfill the varied needs of our residents. The goal of our activities team is to provide a dynamic and stimulating home environment. Both departments provide challenging individual and group programming designed to promote intellectual, physical and social stimulation and rehabilitation.
Medical
The Shawnee Rose Cares Center’s Medical Director visits the facility regularly.

Pathways Rehabilitation
Pathways offers a full rehab experience for a wide variety of adults with need for rehabilitation following a hospitalization after an injury, illness, or planned procedure. Our experienced and dedicated rehabilitation team, including therapists, nurses and social services professionals, work with individuals to return to a safe and independent level of function. Pathways professionals are equipped to meet the needs of most diagnostic situations, including:

The time has come to leave the beautiful people in this place. Having been here before, I was impressed with the new therapy department. Occupational Therapy has helped me learn new ways to put on my shoes and socks and I can do it by myself. Physical Therapy has helped to strengthen my legs and get me back to walking again. I recently was a short-term resident at Polo Rehabilitation and Health Care following surgery. The staff was incredible and deserve special mention. I appreciated the service and, most of all, the friendship extended to me when I was a resident there. All the CNAs tended to my needs. Their gentle and kind attention was exceptional; I am sure it sped my recovery. Those who prepared and served my meals also played a big role. The food was excellent and the cheery smiles of the servers brightened the sometimes stressful days. The Pathways therapists pushed me to stretch those stiff muscles.
I am sure that I would not be doing as well as I am – and be back in my own apartment — without them. Those who made it possible for me to get to my doctor appointments aided tremendously. It was definitely easier for me in the Polo Rehab van than in a car. You gentlemen are true gentle-men. I truly feel that the Polo Rehabilitation and Health Care facility is one of the best available in this area. I cannot thank all the staff enough. It is top notch nursing care.
Well, this was the best decision I could have made for Mom. Every staff person listened to me as though my concerns were their only concerns. They took into consideration all of Mom’s individual needs, even her personal preferences that were not within the normal protocols of the nursing home. They really listened when I talked. They kept me informed. They respected her wishes, through me, since she could no longer express herself.
