All Are Welcome
All Cornerstone Rehabilitation & Health Care Center services are provided without regard to sex, race, color, religion, national origin, or ability to pay. Residents must be age 18 and older and attended by a licensed physician. Eligibility is determined by our admissions committee after a pre-admission conference. Each resident must have a complete history and physical examination prior to admission, and the attending physician also must sign orders for all care and medications.
We provide a kind of care that is so personal, so effective that it requires a special kind of devotion-commitment to human life. It is the philosophy of Cornerstone Rehabilitation & Health Care Center to enhance the functioning of each resident, to prevent further disability, and to help residents continue to live with dignity.
Who We Are
Cornerstone Rehabilitation & Health Care Center opened in 1971 as Galena Park Home as a result of the combined efforts of Peoria’s three hospitals. In January, 2013 Petersen Health Care assumed full ownership of the nursing home. Its original one story design is very convenient for both our residents and for our staff to provide the personal care and prompt attention our residents and families expect.
As a state licensed 98 bed, not-for-profit continuing nursing care facility, we provide both short-term and long-term skilled and intermediate care. Cornerstone Rehabilitation & Health Care Center is certified as both a Medicare and a Medicaid provider and is owned and operated by Petersen Health Care.
Our Long Term Care Facility Services
Our plan is a team approach, using the specialized skills of all the various health-care disciplines of our staff to meet an individual’s unique needs. We encourage our residents, their families and friends to participate in this team approach. By offering the following exceptional medical capabilities, we help make Cornerstone Rehabilitation & Health Care Center the right choice for anyone 18 years and older.
Respite Care
Caregivers in need of a break can call on us to provide temporary housing and nursing care for a loved one, from two days to two weeks.
Intermediate Care
This level of care is for individuals requiring supervision with medication and assistance with daily living skills.
Skilled Care
Available 24 hours a day by licensed practical and registered nurses and certified nursing assistants who meet professional standards. We are equipped with the technology and personnel to administer many day-to-day medical treatments which minimize trips to the hospital.
Rehabilitative Care
Whether the goal is to return home or to maximize overall wellness for Seniors, rehabilitation and restorative nursing play an important part in the recovery process following injury, illness, or surgery. Specialized physical, occupational, speech, and recreation therapy programs are directed by registered therapists in the resident’s room or in our well-equipped rehabilitation department.

Pathways Rehabilitation
Pathways offers a full rehab experience for a wide variety of adults with need for rehabilitation following a hospitalization after an injury, illness, or planned procedure. Our experienced and dedicated rehabilitation team, including therapists, nurses and social services professionals, work with individuals to return to a safe and independent level of function. Pathways professionals are equipped to meet the needs of most diagnostic situations, including:

I recently was a short-term resident at Polo Rehabilitation and Health Care following surgery. The staff was incredible and deserve special mention. I appreciated the service and, most of all, the friendship extended to me when I was a resident there. All the CNAs tended to my needs. Their gentle and kind attention was exceptional; I am sure it sped my recovery. Those who prepared and served my meals also played a big role. The food was excellent and the cheery smiles of the servers brightened the sometimes stressful days. The Pathways therapists pushed me to stretch those stiff muscles.
I am sure that I would not be doing as well as I am – and be back in my own apartment — without them. Those who made it possible for me to get to my doctor appointments aided tremendously. It was definitely easier for me in the Polo Rehab van than in a car. You gentlemen are true gentle-men. I truly feel that the Polo Rehabilitation and Health Care facility is one of the best available in this area. I cannot thank all the staff enough. It is top notch nursing care.
Well, this was the best decision I could have made for Mom. Every staff person listened to me as though my concerns were their only concerns. They took into consideration all of Mom’s individual needs, even her personal preferences that were not within the normal protocols of the nursing home. They really listened when I talked. They kept me informed. They respected her wishes, through me, since she could no longer express herself.
Petersen Health Care was an absolute answer to prayers for our family as we sought the proper facility for our Aunt. The facilities are very home-like, and the care and love given to the residents by the staff is exceptional. I would recommend Petersen Health Care to anyone looking for a home for their loved one with dementia.
The time has come to leave the beautiful people in this place. Having been here before, I was impressed with the new therapy department. Occupational Therapy has helped me learn new ways to put on my shoes and socks and I can do it by myself. Physical Therapy has helped to strengthen my legs and get me back to walking again. If I need therapy again, I will definately come here!
Everyone from housekeeping to the nursing staff is wonderful. I know that my mom is in the hands of wonderful, caring people. When she came there 2 years ago, I wasn’t sure I was doing the right thing, but you have proven over and over again that it was the best thing for her that I could have ever done when I could no longer meet her needs myself. Thank you for taking such good care of my mom
