Admission Policy

The following is a list of documentation needed prior to any admission at our nursing facilities:

• Copy of resident’s Medicare card
• Copy of resident’s Social Security Card
• Copy of all cards related to resident’s insurance (Supplemental and long term)
• Copy of resident’s Power of Attorney (POA) for Health Care
• Copy of resident’s Living Will

When paying privately, we require that one month be paid in advance.
